For someone living with Alzheimer's or dementia, a medical ID helps first responders and strangers understand the situation quickly and get them home safely.
What to include
- Name. First and last name.
- The condition. For example, ALZHEIMER'S, DEMENTIA, or MEMORY LOSS. This tells others the person may be confused or unable to answer questions.
- Emergency contact. ICE (In Case of Emergency) with a caregiver's phone number. Add a second number if you have room.
- Other key conditions and medications. For example, DIABETIC or ON BLOOD THINNERS.
- Allergies. Start with ALGY: (for example, ALGY: PENICILLIN).
Sample layout
Our Alzheimer's / Dementia bracelet fits 4 lines on the front (18 characters each) and 4 lines on the back (20 characters each).

Put the most important details (name, condition, and a phone number) on the front, since that's what people see first.
Tips
- Pair it with our sister hook clasp, which takes two hands to open, so the bracelet stays on.
- Check the spelling and phone numbers twice. We engrave exactly what you enter.
- Need room for more, like a full medication list? A wallet card or a Smart QR medical profile can hold the details, with "SEE WALLET CARD" engraved on the ID.
- Not sure what matters most? Ask your loved one's doctor.
